Clinical_Patient Care Programmes
 
Following admission patients are assessed and individual care programmes are prescribed and discussed with the patients and their relatives and carers. During the course of the patient’s stay in hospice any necessary changes to this programme are explained and agreed prior to their introduction.

Discharge planning meetings are held for all patients prior to their discharge. These meetings are attended by the relevant health care professionals together with the patient and his/her relatives or carers. This enables an appropriate care package to be in place prior to the patient’s return home.

Comprehensive channels of communication are in place to ensure a seamless service between hospital, hospice and community.

The Hospice is committed to providing support and advice for patients’ families and carers.
