The Community Macmillan Nurses are based at the Hospice and together with other members of the Clinical Multi-Disciplinary Team ensure that a seamless patient care service is provided both prior to admission and following discharge. Patients, together with their relatives and carers, also have access to an appropriate level of input from specialist nurses, social services, allied health professionals, lymphoedema nurses, spiritual and religious practitioners and bereavement counsellors.
